Voluntary Benefits

You may enroll in Identify Theft and Pet Insurance at anytime. AFLAC Supplemental Plan, LegalEASE, Voluntary Life and VSP Vision are voluntary benefits that you may only enroll in during Open Enrollment or a qualifying life event. Open Enrollment for Plan Year 2022 is May 1st through May 31st! Please see below for more information.

Accident Insurance – Aflac

Auto and Homeowner’s Insurance – Liberty Mutual, MetLife, Travelers

This program allows you to apply to purchase quality group auto and home insurance with special group discounts. A variety of insurance policies are available through the program including auto, home, landlord’s rental dwelling, condo, mobile home, renter’s, recreational vehicle, boat, personal excess liability (“umbrella”), and fire. Critical Illness – Aflac

When diagnosed with a critical illness such as cancer, a heart attack (myocardial infarction), or stroke, critical illness insurance can help meet out-of-pocket expenses. Hospital Indemnity – Aflac

Aflac group supplemental hospital indemnity insurance plan can help cover what your major medical insurance may not cover. Providing financial assistance to enhance your current coverage so you can avoid dipping into savings or having to borrow to cover out-of-pocket-expenses that health insurance was never intended to cover. Legal Services – LegalEASE

Long Term Care – AGIS

Long Term Care insurance can provide cash for assistance with care at home or in a facility due to a cognitive impairment like Alzheimer’s or dementia. As an active full-time employee working at least 20 hours per week or a retiree you are eligible to apply for an exclusive Long Term Care insurance benefit. As an eligible active employee your extended family can participate including your spouse, parents (in-laws), grandparents (in-laws), employee’s siblings (in-laws) and their spouse, employee’s adult children and their spouse. Long Term Disability – The Standard

Long Term Disability insurance can help pay the bills if you become disabled and cannot work for a longer amount of time. This insurance replaces a portion of your pay when you miss work because of a covered disability. It also provides partial benefit payments after you return to work, and if your disability returns within a period of time, benefits can resume with no waiting period. Pet Insurance – ASPCA and Nationwide Pet Health Insurance

Pet plans from Nationwide offer coverage for vet bills, with 90% reimbursement for accidents, illnesses and more. Get a quote by calling 1-877-738-7874. Vision Care – VSP

The VSP network doctor list includes skilled and certified optometrists and ophthalmologists selected based on their professional licensing, work history, education, malpractice history, professional liability, and ethics. Coverage includes up to $140 allowance on frames with the base plan or up to $220 with the buy-up plan. There is also 20% savings on frame cost over your allowance each year with either plan. Voluntary Life – The Standard

Group policy Voluntary Life insurance is a financial protection plan that provides a cash benefit to a beneficiary upon the death of the insured. Doctor on Demand – (DOD)

Doctor of Demand connects CDHP and Premier (EPO) Plan participants face-to-face with a board-certified doctor or licensed psychologist (by appointment) on your smartphone, tablet or computer through live video.   • If you are a CDHP participant, Doctor On Demand medical visits are $49 per visit. Visits with a psychologist are $79 for 25 minutes and $119 for 50 minutes. Psychiatric 45-minute initial visit is $229 and $99 for a 15-minute follow-up visit.
  • If you are a Premier (EPO) Plan participant, Doctor On Demand medical visits are $10 per visit. Visits with a psychologist are $25 for 25 minutes and $35 for 50 minutes. Psychiatric 45-minute initial visit is $35 and $25 for a 15-minute follow-up visit.

Nevada Public Employees’ Benefits Program partnered with Express-Scripts’ SaveonSP, to help you save money on certain specialty medications. If you take medications found on the SaveonSP Drug List you are eligible for the program and select specialty medications will be free of charge ($0). Accredo and Express Scripts will contact you if you are currently taking a specialty medication, so be on the lookout for future mailings.
