PLAN YEAR 2019
Open Enrollment for the Public Employees’ Benefits Program (PEBP) for Plan Year 2019 is scheduled May 1 through May 31, 2018. Changes to your PEBP health care benefits or to your dependent coverage tier may be made within this limited time frame. If you do not wish to make any changes, no action is required.
If you are currently in the North and on the Hometown Health HMO, you will automatically be enrolled in the Premier (EPO) Plan for Plan Year 2019 unless you make a new plan selection through the E-PEBP Portal or by form.
Open Enrollment is the only time of year you can make changes to your health benefits without experiencing a qualifying life event (e.g., marriage or birth of a child). Any changes you make during this Open Enrollment period will be effective July 1, 2018.

If you are planning to enroll in Flexible Spending, complete this form and fax to HealthSCOPE Benefits by May 31, 2018 at 1-877-240-0135.
If you are adding dependents, all supporting documentation is due in the PEBP office no later than June 15, 2018. You can either fax 775-684-7028, email (firstname.lastname@example.org) or mail your supporting documents to 901 S. Stewart St Suite 1001 Carson City, NV 89701.

Information for Retiree Late Enrollment
Retiree Late Enrollment is the time when retired state employees and retired employees from a PEBP participating local government (or the surviving spouse or domestic partner of such retirees) can reinstate PEBP retiree coverage one time. Via Benefits Medicare Exchange Retirees
Retirees and their eligible spouses/domestic partners enrolled in a medical plan through Via Benefits (formerly Towers Watson’s OneExchange) may newly enroll in or opt-out of their current PEBP dental coverage during Open Enrollment. To make changes online, log into your E-PEBP Portal at www.pebp.state.nv.us. You can also request an Open Enrollment Form by calling 775-684-7000 or 1-800-326-5496. Completed forms must be postmarked by May 31, 2018. If you do not wish to make any changes, no action is required.
For Plan Year 2019, a one-time, lump-sum contributions will be provided to all eligible retirees covered on a medical plan through Via Benefits as of July 1, 2018, or who are eligible for the HRA due to having TRICARE for Life as of July 1, 2018. The funds will be deposited into the Via Benefits HRA accounts within the first few weeks of July.
